Passport Application Instructions

Delays in Overseas U.S. Passport Processing

The Department of State’s Bureau of Consular Affairs is currently experiencing technical problems with our overseas passport and visa systems.  This issue is not specific to any particular country, citizenship document, or visa category. We apologize for the inconvenience and are working urgently to correct the problem and restore full operability.

U.S. passport applications accepted overseas or after May 26, 2015 are affected by this delay.  If you applied for a U.S. passport overseas during this time frame and have travel plans within the next 10 business days, please consider requesting an emergency passport at the U.S. Embassy or Consulate at which you originally applied.  Information about how to apply for an emergency passport is available at the following

Adult Renewal

If your most recent passport:  was issued after your 16th birthday; was issued less than 15 years ago; is currently in your possession; has the same name you currently use

  • Applicant’s most recent passport
  • Fee of $110 payable in either ariary or dollars (cash and credit card are accepted)
  • Completed and signed form DS-82

If your most recent passport: is lost or stolen OR was issued before your 16th birthday

  • Applicant must be present at the Embassy
  • Applicant’s most recent passport (if it’s in your possession)
  • Birth certificate/CRBA or other proof of identity (if your passport is not in your possession)
  • Fee of $135 payable in either ariary or dollars (cash and credit card are accepted)
  • Completed form DS-11 (do not sign until you are in front of a consular officer)
  • Completed form DS-64 (if your most recent passport was lost or stolen)

Minor Passport Issuance and Renewal (for children under the age of 16)

  • Both parents and the child must be present at the Embassy (if one parent absolutely cannot be present, form DS-3053 must be submitted)
  • Child’s birth certificate/CRBA
  • Both parents’ passports
  • Copy of the parents’ marriage certificate (and termination of prior marriages certificate, if applicable)
  • Photos of the child growing up since last passport issuance (if the application is a passport renewal)
  • Fee of $105 payable in either ariary or dollars (cash and credit card are accepted)
  • Completed form DS-11 (do not sign until you are in front of a consular officer)

Additional pages

Presence at the Embassy is not required but both parents have to sign for minors. Only 2 inserts are allowed for a passport. Photo is not needed.

  • Applicant’s most recent passport where you want to insert the pages
  • Fee of $82 payable in either ariary or dollars (cash and credit card are accepted)
  • Completed and signed form DS-4085


 All passport applications require 2 photos (2” x 2”/ 5cm x 5cm) in color with a white background
